1. Do you understand that even with a disability, this cat can live 15+ years and are you
ready to make that life long commitment? Yes no
2. In the event that the adoption does not work out, or you are no longer able to care for the
cat, do you agree to return it to Kitties from Heaven? Yes no
3. Do you agree to keep your animals vaccinations current and provide it with the necessary
medical treatment? Yes no
4. Are you aware that this cat will need more specialized medical care than a normal healthy
shelter animal? Yes No
5. If required by the city in which you live, do you agree to keep your animal licensed, and
pay all applicable licensing fees? Yes No
6. Do you agree to have your animal spayed or neutered (if applicable) and provide proof of
the procedure to Kitties from Heaven? Yes no
7. Do you agree to keep a means of identification on your cat at all times (I.e. collar and tag,
microchip, etc.)? yes no
8. Do you agree to provide all necessary and reasonable medical care to this special needs
animal to give it a good quality of life? Yes No
9. Do you agree to contact Kitties from Heaven if you are not able to provide care, due to
cost, prior to making any decision to euthanize the animal? Yes No
10. Do you agree that, if there is any breach in this contract, you will return the animal to the
shelter at no charge to you? Yes no
11. Do you understand that Kitties from Heaven is not responsible for the medical condition
from which this cat suffers, and do you understand that you are adopting a cat with a
specific medical need that may require extra care and cost? Yes No
I, the adopter, agree to the above contract and agree to provide my new cat with all of the
necessary love and care. I agree to provide it with food, water, shelter, and medical
treatment. I also agree that I will not hold Kitties from Heaven responsible for any property
damage or personal injury resulting from this adoption or possession of this animal. I also
understand they by failing to sign this contract or by failing to abide by its terms that the
adoption becomes null and void.
